Commonwealth Care of Roanoke, Inc. based in Roanoke, VA is currently searching for a dynamic Traveling MDS Coordinator to support our centers and our team of dedicated Center MDS coordinators as part of a dynamic Corporate MDS/RAI team. This position supports our 12 centers across the state of Virginia. This position is a remote and/or onsite position but will require extensive travel at times, including overnight, to assigned centers. Whats My Primary Role
Understanding the wide financial and regulatory impact of adequately and accurately capturing all services provided to or needed by patients and the potential negative financial, regulatory, and legal implications to the organization and individuals if this outcome is not achieved.
Communicate frequently with Regional Clinical Reimbursement Specialist assigned to the center as well as other Center MDS staff regarding status of active assignments.
Making informed decisions regarding MDS item set coding judgments based on the clinical data available to accurately reflect the patient status, especially when some data may be conflicting.
Complete portions of the MDS, CAAs, and Care plans and CCR required supporting UDAs or notes as required to ensure timely and accurate completion.
Coordinate and participate in the completion of MDS assessments, triggers, CAAs and comprehensive plans of care via review of the residents medical record, communication with and observation of the resident, communication with direct care staff, communication with the residents physician and physician extenders and communication with the residents family and Interdisciplinary team.
Assisting the Center in determining and assessing level of care for new and ongoing residents in accordance with Skilled Care Criteria as outlined by federal, state, and local payers.
When required, coordinate with all disciplines the establishment of the most appropriate common assessment reference period to accurately reflect the patients status and resources necessary to care for the patient.
Ensure that interdisciplinary assessments and care plan development and updates are completed using common assessment periods within the time frames established by State and Federal guidelines and Commonwealth Care policies.
Ensures the interdisciplinary assessment team utilizes appropriate resident assessments and triggered CAAs in the development of a comprehensive, interdisciplinary plan of care reflecting the residents current physical, mental, and psychosocial needs.
Ensures that documentation related to the resident assessment process is maintained according to Medicare/Medicaid, State/Federal Guidelines, and Commonwealth Care policy and procedures. Notify Supervisor, Center Administrator and DON of concerns when identified.
Reviews completed MDS information prior to assessment completion for accuracy and completeness, verifies the MDS assessment using the approved MDS scrubber software and responds to/corrects issue alerts before completing and submitting the MDS.
Follow CCR standards set for accurately coding MDS sections remotely to comply with Federal regulations.
If an RN, attests to the completeness of the MDS through signature (Z0500).
Works with the Regional Clinical Reimbursement Specialist, Administrator and MDS team in the center to assure CMI scores are an accurate reflection of the facilitys residents using corporate tools for the accuracy measurement.
Demonstrate the ability to assess, plan, implement, and evaluate all aspects care; including physical, mental, and emotional status.
Demonstrate understanding and ability to assist in center level month end Triple Check process to ensure timely month end close out and billing.
Demonstrate a working knowledge of SNF Quality Measures and Indicators impacted by the MDS
When working onsite:
Demonstrate the ability to communicate effectively with family members regarding residents care and/or condition.
Demonstrate the ability to address complaints, follow up and keep administration informed.
When required, attend clinical morning meeting, morning stand up meeting, at risk meetings and other resident related meetings as appropriate or directed by supervisor to develop an appropriate plan of care, or make revisions to the existing care plan based on the changes in the residents condition.
Demonstrate the ability to gather pertinent information form the PPS/Clinical Reimbursement portion of the morning clinical meeting and Utilization Review group by coordinating with the team members to discuss ongoing skilled care needs, progress and discharge disposition of all Medicare Part A patients.
Perform other duties within the scope of the education and experience as assigned.
Do You Have What It Takes?
Must maintain a current registration or license with the Virginia Board of Nursing as licensed RN or LPN.
Several years of extensive experience with the MDS 3.0 completion and processes required.
RAC-CT certification required within one year of employment if not certified at the time of hire. Must maintain certification as current.
Proficient in Microsoft Office, (including TEAMS, Work, Excel, Outlook).
Must be able to meet deadlines and work well under pressure to produce accurate assessments and other documentation completion requirements whether onsite or working remotely.
Must have transportation and valid drivers license to travel between centers as necessary.
Must be flexible, detail-oriented, organized, have the ability to multi-task and be able to make effective decisions and work as part of a service excellence team.
