A health services network in New York City is currently seeking a bilinguallicensed Registered Nurse (RN) to join their staff as a UAS RN. In this field role, the Assessment Nurse will be responsible for doing UAS assessments in a tight area in Brooklyn. Responsibilities: The Assessment Nurse (RN) will be responsible for: Performing home visits to conduct functional, psychosocial and environmental safety assessments Determining eligibility of enrollees Establishing an individual plan of care that enables them to remain safely at home despite long-term care needs Ensuring that DOH UAS information is complete and accurate Conducting follow-up assessments to ensure the members are receiving the proper care and services. Performing other duties, as needed Qualifications: Prior UAS, MLTC or home care experience Bilingual in Russian Desired Skills: Bachelor's Degree in Nursing
