Registered Nurse (RN)

Middle Island Adult Day Health Services is looking for a talented and hardworking RN (Registered Nurse) to join our ever-growing team. The ADHS is a medical model day program providing medical care and socialization to those living in the community, while also offering respite to their caregivers. Our community-based program allows individuals to remain at home and in their community.

Registered Nurse (RN) DUTIES AND RESPONSIBILITIES

The Program Nurse RN (Registered Nurse) shall provide nursing services and evaluate the need of each Registrant for nursing care and coordinate continuing services.

• Responsible to coordinate program activities and personnel assignments.
• Provides leadership in Health Education programs. Develops and implements such programs for Registrants and caregivers.
• Monitors Registrant's health status through skilled and nonskilled nursing interventions.
• RN (Registered Nurse) Responsible for appropriate documentation such as, but not limited to, care plans continued to stay, medication and treatment records, graphic records etc.
• Actively participates in quality improvement strategies.
• Supervisory responsibilities include but are not limited to, general staff supervision; job performance appraisals, monthly reports, meeting attendance, leadership, etc.
• Responsible for the coordination, provision of, or arranging for physician/medical services, nursing care, social services, nutritional services, recreational services, rehabilitation services and ancillary services through current physician orders
• Responsible to maintain compliant health record and general records according to ADHS policy and procedures.
• Demonstrate ability to coordinate and collaborate with resident/family and other members of the comprehensive care team.
• Maintain a Person Centered care plan for each individual.

Registered Nurse (RN) QUALIFICATIONS/ REQUIREMENTS

• Graduate of an accredited school of nursing
• Registered Nurse
• Nursing experience which one has acquired clinical competence and strong leadership skills
• Ability to be self- directive in delegated areas of responsibilities.
• Hours 8am -4pm
• All Major holidays the facility is closed

Salary: up to $53 an hour
