QUALIFICATIONS:

High school diploma or G.E.D required. Current Licensure in the State of Idaho as a Social Worker, BSW required, MSW preferred. One year of experience in community-based primary care is required. Demonstrates the ability or commitment to understanding the Coeur d’Alene Tribe’s culture, community, and the historical and intergenerational trauma affecting tribal members. This position requires cultural sensitivity and the ability to engage respectfully with Native American patients and families experience required. Typing skills of at least 35 wpm preferred. Current BLS required and/or willingness to obtain within (6) months of hire required. Must have received or be willing to receive COVID19 vaccination per current hiring policies.

ADA ESSENTIAL FUNCTIONS:

  • Hearing: within normal limits with or without use of corrective hearing devices.
  • Vision: adequate to read 12-point type with or without use of corrective lenses
  • Must be able to verbally interact with staff, clients and the public
  • Manual dexterity of hands/fingers for writing, computer input
  • Able to lift up to 25-100 lbs. (assist in transfer of patients)
  • Stand 40% of the day.
  • Walking 60% of the day.
  • Pushing, up to 50-300 lbs., (i.e. crash cart, stretcher, or wheelchair)
  • Pulling up to 25-80 lbs.

RESPONSIBILITIES:

  • Provides social work services within the primary care setting, focusing on care coordination, prevention, and support for patients across the lifespan—with particular attention to the needs of elderly patients and families
  • Functions as a dependable team member by demonstrating self-initiative when needed and assisting when requested.
  • Promote continuity and consistency to deliver quality care.
  • Manage chronic care needs as well as health maintenance, meeting clinic expectations for health maintenance standards
  • Access resources for referrals as needed and schedule appointments for patients needing referrals.
  • Promotes care aligned with clinical best practices
  • Acts as a coach/resource to clinical staff
  • Participates in development of case management in the Patient-Centered Medical Home model
  • Leads or supports training of new social workers
  • Demonstrates ownership of issues and contributes to projects
  • Remains calm and effective in emergent or changing environments
  • Collaborates with interdisciplinary teams (primary care, nursing, behavioral health, pharmacy) to develop comprehensive care plans
  • Counsels and provide crisis intervention for patients.
  • Participates in and contributes to the discharge plan for patients that are hospitalized
  • Arranges discharge follow up appointment and phone call from RN after patient is home
  • Arranges and verifies that patient has all needed equipment and supplies prior to discharge
  • Advocate for patients and ensure they are receiving culturally appropriate care.
  • Assist providers with ACP and introducing patients to advanced directives and living wills.
  • Provide education to seniors and their families on topics such as healthy aging, end-of-life planning, and managing chronic conditions
  • Ensure patient education/teaching plan by patient needs.
  • Arranging for resources /funds to finance medications, DME and other needed services.
  • Ensure complete, accurate and timely documentation within the electronic health record including the right patient, right encounter date/time and appropriate provider.
  • Able to accurately and completely fill out an ROI.
  • Transfer information is documented accurately, completely and in a timely manner for patients’ transfer to other health care facilities.
  • Use electronic health records and/or develop tracking systems to track, monitor, and assure the appropriate follow-up of patients.
  • Conducts home visits for elderly or high-need patients and may provide support to patients in local hospital settings as needed.
  • Conducts psychosocial assessments during home visits to evaluate safety, support systems, and mental health status
  • Provides oversight of Home Assistants, ensuring completion of home safety checks and assisting with the development and implementation of individualized care plans.
  • Monitors and communicates changes in patient symptoms and needs
  • Provide emotional and practical support to elderly patients, family members, and caregivers
  • Respects and supports the patient’s rights to treatment or services in accordance with clinic policy.
  • Promotes quality of life, independence, and informed consent for aging populations
  • Identifies needs for DME (e.g., oxygen, wheelchairs, medical alert systems)
  • Reports suspected abuse or neglect in accordance with guidelines
  • Connects patients with community resources and elder care support
  • Build partnerships with senior centers and local organizations.
  • Practices Universal Precautions, Infection Control and Safety Measures.
  • Follows established policies for safe administration of medicines, treatments, and procedures.
  • Meets mandatory education requirements for safety, i.e. blood borne pathogens, body mechanisms, hazardous chemicals, fire and safety procedures.
  • Communicates situations of potential legal risk at times of occurrence through an appropriate chain of command process.
  • Able to utilize all medical equipment within scope of practice safely and appropriately.
  • Other duties as assigned.
