Job Description
The Full Time Social Worker at Unicity Senior Advisors will provide comprehensive support and assistance to elderly clients. Your primary responsibility will be to manage a caseload of clients, ensuring they receive the appropriate resources and interventions needed to improve their quality of life. This role requires collaboration with clients, their families, and various healthcare professionals to create and implement personalized care plans.
Your key responsibilities will include:
Conducting thorough assessments of clients' physical, emotional, and social needs.
Developing and implementing individualized care plans in collaboration with clients and their families.
Providing counseling and emotional support to clients and their families.
Coordinating services and support through community resources and healthcare providers.
Monitoring clients' progress and adjusting care plans as necessary.
Maintaining accurate documentation of client interactions and services provided.
Building and maintaining relationships with community organizations for resource and referral purposes.
Participating in team meetings and professional development opportunities.
Requirements
Qualifications and Requirements:
Bachelor’s or Master’s degree in Social Work (BSW/MSW).
Active and valid social work license (LSW or LCSW) in the state of New Jersey.
Experience working with elderly populations and understanding of geriatric care.
Strong interpersonal and communication skills.
Ability to work independently and as part of a collaborative team.
Valid Driver’s License and reliable personal vehicle.
